Ultimate Workout Registration Confirmation

Thank you for submitting your registration for Ultimate Workout 25! We’re looking forward to serving together.

Confirmations notifying applicants of their acceptance on the project will be sent out as soon as possible. Along with your confirmation, you will receive additional forms and the flight guidelines. Please do not purchase your airfare until you are confirmed.

The following items must be received at Maranatha Volunteers International online or by mail six weeks before the project:

1. Full payment. $150 becomes non refundable one month before the project. Some exceptions may apply. If you need special payment arrangements, please call our office and talk with a volunteer projects specialist
2. Legible photocopy or scan of your passport (UW25 – Panama Only)
3. Legible photocopy or email (preferred method) of your complete flight itinerary with confirmation code listed: uw@maranatha.org
4. One original notarized “Permission for a Minor to Travel” form for those under the age of 18 years of age on the first day of the project. (Keep the second notarized form with minor’s travel documents/passport)
5. Three recommendation forms
